We're seeking a compassionate and collaborative Registered Nurse to join our client's PACE (Program of All-Inclusive Care for the Elderly) team. This is a unique opportunity to work with a passionate interdisciplinary group, delivering high-quality, integrated care to frail older adults across multiple care settings—including the clinic, adult day center, home visits, virtual care, and skilled nursing facilities (SNFs).

What You’ll Do:
Collaborate with the Interdisciplinary Team (IDT) to develop and execute individualized care plans
Conduct comprehensive face-to-face nursing assessments (physical, psychosocial, and behavioral)
Deliver direct nursing care in line with physician orders—administering medications, therapies, and clinical interventions
Act as a triage nurse in outpatient settings, initiating care within your scope and escalating appropriately
Provide case management support during transitions of care (hospital, SNF, home)
Educate participants, caregivers, and team members on care plans and best practices
Rotate in taking after-hours calls (triaged through nurse team)
What We’re Looking For:
Active California RN License in good standing
Passion for caring for the elderly and a holistic, team-based approach to healthcare
Experience in geriatric care, home health, SNF, or PACE a strong plus
Excellent communication, documentation, and case management skills
Adaptability to work across various care settings
Strong commitment to empathy, integrity, and continuous learning

Additional Notes:
Registered Nurses must have experience in geriatric care, bilingual in Spanish is highly preferred. Our client is also looking for actively working RN's with recent hands-on care.
This is a clinic-based role, although there can be a chance this RN will see a patient in their home, but mostly daily clinic visits. Must have strong clinical acumen - from CNA, to LVN, to RN. This is not a case management role. However, the responsibility of case management is about 20-30%. We’re seeking nurses who can provide bedside nursing, clinical interventions, med box, starting IV's, etc. Our client is NOT looking for remote, telephonic or Case Management nursing experience.
Work Location: 121 S Mountain View Ave, 90057.Greater/Downtown LA office. (Secure, free parking garage). Candidates must live within a 10 mile radius of Los Angeles
Compensation:
$50/hr. + $15,000 sign-on bonus
